ESA50 completion

After three years, I have been sent another ESA50 to complete.

I have tried reading through the notes provided but am unsure what to tick! If for example I can walk up and down two steps but it causes me a great deal of pain to do so should I tick that I can do it and explain its very painful to do so or tick no?

If you do not feel that ticking any of the boxes accurately reflects your limitations then we suggest that you don't tick any of them, write 'see below' ( or 'see additional sheet x' etc) and explain the situation in detail.
