G.P appointment advice

I have a GP appointment in 2 weeks time. I very rarely see them as they are not helpful. I would like to use the appointment to update them regarding my health problems for the DLA to PIP changeover.
Its only a short appointment so i was wondering are there any template letters or forms i could complete listing my health problems and difficulties and just hand over for inclusion in my medical records?
Any advice how best to achieve this?

I don't think we have any temp[late letters for this, everyone is so different. You could take each PIP descriptor that you think applies to you and write a paragraph explaining how you are affected but I would expect this to take you quite a lot of time.
