PIP & 28+ Day Hospital Stays

I recently helped someone apply for PIP, the recipient was awarded enhanced rates for both components. The recipient's health is particularly bad at the moment, a 28+ day hospital stay is possible.
As I understand it, PIP payments will stop if the recipient is in hospital for more than 28 days. Whose responsibility is it to inform the DWP of the hospital stay, the recipient's or the hospital's? And, more importantly, what happens when the recipient returns from hospital - are PIP payments simply recommenced, or is a new PIP application required?

As always, it is the claimants responsibility to inform the DWP if they will be in hospital for a period of 28 days or more or a total of 28 days in two or more periods as an in-patient separated by 28 days or less.
Payments of both components will be suspended while the claimants meets the above but will be re-instated when they do not.

You don't need to inform them unless you have or expect to be in hospital for more than 28 days in a single period or a linked period.
