Hi,
I have tried a search but would ask the following do you need to attend the pip assessment? My condition both physical and mental ground make it impossible for me to know if I can attend on that specific day for instance if it was today I could not attend its behond my control!
Also this walking 20m condition also prohibites me from attending unless the assessment examination room is within this distance from the car I can't attend.

In the recent newsletter were is the PIP form to down load All help appreciated

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ
My full name is showing, how can I stop it?
If an assessment is deemed to be required by the Assessment Provider (AP) then you will need to attend or your claim will be closed, however, it is possible for the AP to make a recommendation on your completed form and evidence, it also possible for a home assessment to be done, you will need your GP to write to the AP explaining why you cannot attend at their offices.
The report is in the PIP section.
