confused about which E.S.A form guide to use was created by Annabel
Hello,
I have searched the FAQ on E.S.A and forum about which E.S.A form guide to use but am confused about it.
I have recently received a new E.S.A form dated 04/15 to complete. I was initially assessed for E.S.A about 3 years ago, and a decision was made in May 2012. I was transferred from Incapacity Benefit to E.S.A WRAG group. Does this mean I need to use the most recent, new E.S.A form guide 2015 for my re-assessment or the 'old' E.S.A form guide before 28 January 2013? Will I be assessed on the new criteria? I would be grateful if someone could tell me which guide to use?

You need to use the new form guide.
You can use your old form if you kept a copy to ensure you transission all your information across nd you will need updated evidence from your GP etc. to support your answers to the activities questions. Just remember the claim is all about how your condition(s) affect you in relation to the activities so concentrate on that and compare your situation now with how you filled in the form 3 years ago to ensure continuity.

Have you thought of : Using The PDF Version Of The ESA50 Questionnaire whilst referring to the guides on this page, 2nd & 3rd links down ? :

A lot of our members have and find it much easier !

Just wanted to say thank you very much for all your advice. I did use the pdf version for the esa form and it was easier!
