Do I send a copy of my medical report along with my appeal?

I think the best idea when considering appeal is to gather together all the recent supportive medical evidence you have and send it along.

If you read the guides to appeal you will be able to decide for yourself what is and isn't necessary.

You need to decide if the medical report provides any supporting evidence to your replies to the questions on the form. If it is just related to your medical condition(s) it probably isn't relevant as it is how your health affects you in carrying out the activties that gets you points.
