Assessor blatantly lied on PIP report

When I received the copy of the report I was amazed to see the nurse had said my memory and concentration were 'adequate' and that I did not need prompting. The decision maker has used this report to make the decision.
Ther are various blatant lies on the report including that the nurse did loads of examination of my arm movements. She asked me to do 3 things with my right arm and just asked if I could do the same on my left. According to her she tested my arm strength, that I could put my hands behind my back etc. None of these things were done.
I have been awarded standard for both parts but I am 1 point shy of the enhanced rate for daily living. I am going for mandatory reconsideration as my pain doc has even said on his letter that the pain affects my concentration and memory, but this has been ignored in favour of a 90 minute assessment with a nurse who just makes stuff up.
Help!!!!! I don't want her to get away with it!!

same thing happened to my hubby, report said he rotated hips and ankles and put his arm behind his back, he never did any of these only raised his arms and squuezed her hands, he has parkinsons disease and suffers from falls she said he should be refered to a falls clinic his clinic is a movement disorder clinic he is under a neurological specialist and parkinsons nurse sees them regular she stated he gets no support and onl visits them 4 times a year, hes been 7 in the last year, oh and she also put that he cleans his bottom with his left hand......nothing was mentioned about wipeing his bum,
