I would expect it to be from when the claim is made, future awards will be from the previous Decision Date.
Forms are normally sent out 1-2 months in advance, but you don't need to worry too much, ESA is an indefinite award, so she will continue to receive her current ESA award until a new Decision is made even if this takes her past the 18 months.

Hi h1,
As Gordon says, ESA is awarded "indefinitely" with regular re-assessments !
So, you do not have to contact DWP to instigate a re-assessment.
When the time comes, you should receive an ESA50 Questionnaire through the post.
In anticipation of receiving the hard copy ESA50 through the post, you could put yourself ahead of the "game" by making a start now.
