No one here can actually physically help you fill in the form;- we can't tell you what answers to give, if that's what you want. if you read the guides published here, you will find all the information you need to complete the PIP application.
If you have difficulty with actually writing/reading the form (bad eyesight, physical damage to your hands etc) you could always approach (say) the CAB or DIAL, who may be able to physically help you.
