Hi....Thank you for your reply. As I said I have the envelope so I have been travelling around the site looking for help to complete the forms.
I came across a Consultation report PA4 V3 Phillipa Johnson which I took to be a face to face assessment.
It was very interesting to read and I wondered what the outcome of this assessment was?
I could not find out whether this person was awarded anything or points scored. Do you know?
I have had DLA indefinate since 2001 and never had a face to face assessment. So this was an eye opener for me. The detail of information that was recorded ...scary

It is an example report constructed from feedback from members who supplied copies of their assessment reports. There was no Decision.
If you want to see what other members have been awarded then go through the forum, topics with a heart icon next to them indicates an award.

Thank you for your last reply. I am going through my letters etc., Is there a time limit on the dates of Hospitals/Consultants letters that I can use as evidence?
As I said previous I have not been assessed since I got my award in 2001. Although my letters do not go back that far, they contain a diagnosis plus other details.

Ideally your evidence should be less than two years old. Back in the real world, very few claimants have evidence that is this new so you need to send in what you have and explain why it is still relevant, in part this will depend on the condition, those that do not improve or deteriorate are easier to explain than those that are variable but you can still show that reports are relevant to you limitations as they are now.

Hi...If I were to get a telephone call asking about my conditions, Will they expect me to complete the form over the telephone?
Will they be random questions from the form? or something else.

It's not impossible that you will get a phone call, but it would be very unusual for this to happen.
Technically an assessment can be done over the phone but we haven't heard of one being done in this way for several years.
If they do phone it will to ask basic questions about your claim, but to confirm this is unusual thing to happen.
