i am filling in the form for my wife but because of my own problems i have had help from a friend. they do not want me to put their name on the form. what can i do.
So long as your wife signs the form, indicating that everything on it is true then there is not really any need to worry. Many people find the form daunting, or can't fill it in for physical/mental health reasons. Don't worry.
