How do I request copies of my claim forms for ESA and PIP as I did not photo copy them before sending them and now want the decision looked at again but need copy of my completed claim forms. Please can anyone help???

If the claims were recent, less than six months, then you may be able to request them from the DWP offices that dealt with the claims, otherwise you will have to issue a Subject Access Request.
