PIP appointee

Hi,
I am in the process applying for PIP for my 16 year old my autistic son ( presently receiving DLA) and I am his appointee. Do I sign the declaration at the end of the PIP application or does he? I wish they would make these points clearer. Just want ot do the right thing.

If you sign and print your name on the form and put a note to explain that you are your son's appointee there shouldn't be any problems. OK?
