Another ESA50 Form

Hello,

I have mental and physical disabilities... and I have received another ESA50 form in a white envelope this time, and I'm stating to feel scared and suicidal again... I don't know who to turn to, and I have no family or friends to support me, I just can't go through with this again, I have to go through all this again on my own... I have been to three ESA reassessments in the past, and each time I have been placed in the support group for three years, but they keep sending me the ESA 50 form. I dread another ESA reassessment because the last time I attended I was totally be belittled, stripped of my pride and dignity, treated like I was nothing, and I ended up crying and then I fainted, needless to say there was no compassion.

Very sorry about all that, I just needed to release my inner fears.

I need to ask some questions that will make the process of filling in the ESA50 form a lot easier for me this time.

And my questions are this:

Could you 100% verify that I can download the ESA 50form from here:

And then can I type in my answers on my pc, and then print off the ESA 50 form, and then send it to Maximus.

Or do I need to write the answers myself into the ESA50 form Maximus had sent me.

It would be so much easier for me to type the answers on my pc and print it off, and do a bit each day and save it.

I just need to be a 100% percent sure I can do this.

Thank you for the guides they are helping me enormously and are a great comfort to know that someone understands, It is like someone is actually there with me while I am filling in the ESA50 form.

The whole basis of the welfare reforms was that all claimants would be reassessed periodically rather than ;languishing on benefits' (their words, not mine!). 3 years is the longest ESA award that we have heard of in normal circumstances.

Yes, it is perfectly acceptable to use the PDF version of the form and fill it in on your computer.

Hello, thank you for your past help. After waiting for four + months! I have finally received my appointment for another Maximus medical this morning. The strange thing is that for my past medical appointments, I have waited no longer then two weeks, they have also gave me the unusual appointment time of 4 P.M. and which wont be for yet another three weeks. The torment from the DWP is unreal... My illnesses have worsened. I will keep all up to date.

Hello again, just an update...

I asked Maximus for a audio recording using your most excellent letter template from the B&W guides, (Thank you!). For me personably the most important reason why I asked for a audio recording was so they could not degrade and humiliate me again... as the evidence will be there on tape if I need to make a legal complaint to the appropriate human right authorities and such like. And so now they have granted me a audio recording, I now feel much more at ease when I eventually go to the Maximus medical. And for a change I now have Maximus on the run.

My original appointment for a Maximus medical was for tomorrow afternoon 4:P.M. - 16th - November - 2016. But because I asked for a audio recording, I received two letters from Maximus this morning, one letter stating they have cancelled my original Maximus medical, the other letter stating that another appointment has been made for me in another three weeks time, and on the day that is my birthday too. But to be honesty with you, I'm glad I have to wait another three weeks, as I will have a little peace of mind now, and besides it was a blessing in disguise, as I was not up to going to the original Maximus medical tomorrow, as I feel much to sore and weak right now and lack of sleep through months of extreme worry.
