Completing the PIP Form - DLA to PIP

Do I fill it out as if I am him e.g. "I have to take medication prescribed three times per day" or do I fill it I out as his parent e.g. "Joe has to take medication prescribed three times per day" and put a comment on the form that explains that I have completed it on his behalf?
I have got to get this submitted by this Friday, can anyone please give any advice on this and any of the questions on the form

If you are using his words, which from your post seems unlikely, then you should use "I" and he signs the form, if you are using your words then "he" and you sign the form, in either case you need to explain how the form was completed.
