I finally received my HA report from DWP. I am astounded at the assumptions inferred from the informal observations and the 'embroidery' - could be worse from what I have read.
It is littered with a lot of abbreviations and I see you have dealt with most of these elsewhere.
I am not sure what FH is - can it be family history? surprised if so because this seems to mean so little in mental health now - it is all mindfulness?
However the HP signed herself as a Nurse/DA- any idea what DA is - thought she was a paramedic?
Your help as always most appreciated.

I can't find anything for Nurse/DA you I suspect it means Disability Assessor or some such reference.
"FH" could be family history but that is not on the standard list of questions, so I'm doubtful but cannot think of anything else it could mean.

The other use of 'FH', in my experience, is to denote 'foetal heart' - I'm assuming this does not apply to you??

FH appeared a lot on my assesment report and I took it to be Functional History.

Thank you both for your help.
No Mrs H, 'foetal heart' does not apply to me.
From reading something just now, it appears to denote Functional History?
(maybe deduced from History of Conditions?)
Amazing to me how in the space of time so much evaluation
can be determined???

I still await result of Mandatory Reconsideration for PIP made early Oct 2016? The interminable waiting does affect me. Around this time I completed IS10 for Enhanced Disability Premium (on ESA) on advice of CAB because of award of PIP Personal Care.
Today, I rang to enquire about the latter to be told said form had been received and a note put on it to say I do not qualify because my ESA is CB? DWP did not inform me. The enquiry centre ESA today did not seem to know either and are due to phone back tomorrow?
I have checked FAQ and Site for clarification and feel confused still. It seems this premium is means tested and would only be payable if you are in receipt of ESA IR ie not only ESA CB? but financial questions were not asked on IS10?
Any help on this will be most appreciated.
