ESA50 HELP!!

Hi
New to this site so apologies if this post is in the wrong place or has already been covered.
I've received a ESA50 capability for work questionnaire. I'm filling it in for my 28 year old son who has Down's Syndrome. I'm his appointee.
I last filled in a face to face assessment form in September 2013. Is ESA50 form the same as that one?
My son is in the income related ESA is that the same as the support group?
This form seems so much smaller. I have to send it back to Centre for Health and Disability Assessments in Croyden, is this the usual place as I seem to remember a different address that I read somewhere??
So confused and so tired of having to fill in these forms!!

Income Related ESA is the type of ESA that your son is receiving.
The Support Group is the current award.
It's possible that you have been sent an ESA50A, this only looks at whether he meets the SG criteria rather than the ESA50 which also looks at whether he meets the WRAG criteria as well.
The ESA Claim guide covers both.

Assessment used to be done by ATOS they are now done by Maximus, also known as CHDA or HAAS.
Whichever form you have received, this is a full reassessment of his claim and you need to complete the form in detail as if it is a new claim. Send supporting evidence as you can, even if it has been sent before.

Thanks for the quick reply. On the bottom of the form is ESA50 04/15.
Not sure I understand about the SG or WRAG, but will read the guides to see if it makes any more sense!
Fortunately I copied the last form and not much has changed since then-worse if anything!
