Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

I would start by reading our PIP Claim guide on the following link

i spoke to my gp yesterday and he says he will support us but wouldn't give a letter, he said he will wait for DWP to get in touch. The only Care Plan I have is from 2008, shall I send that in?

i have filled the form in but do not have an up to date care plan, my gp says he doesn't provide letters but he waits for the official form from work and pensions. He says that he will support the claim because i made him aware of how the condition affects my husband. Doesn't sound very hopeful does it

I'm afraid it is unlikely that your GP will be contacted by the DWP.

Is the Care Plan still in operation, if so then it would be reasonable to include it.

It you have had treatment in the last few years then this should have been reported to your GP and you can access this information.

been for an assessment today, only took an hour, is this a good sign or bad sign, she just seemed to skim over everything, didn't go in to any detail at all, just have to wait and see now, not very hopeful
