When they assess which descriptor is applicable for this question, do they consider that if you can cook a simple meal once a day (mw or cooker) that that's ok or are they looking to see if you can cook a simple meal twice a day (mw or cooker)? I mean, we're supposed to have lunch and dinner in a day but do they just want to know whether you can manage this lunch or dinner?

By default they will consider your preparing and cooking the meal once a day, if there is a requirement for you to have a cooked meal more often than this then, for example due to dietary advice, then you can argue that the test should take into account you ability to do it more than once.
