ESA M12 04/13 form was created by alan
I have been put in the esa support group . today I have received a MI12 04/13 form about getting some help on my mortgage . do I have to fill this form in as I only have a small mortgage and I am not wanting to claim any money towards it .

It's entirely up to you. I would respond to the DWP and say you do not want to make use of the facility.

would sending the form back saying I do not want to make use of this facility effect my esa benefits

I can't see why, they are offering you extra money, so your not taking them up on the offer should not be an issue.
