Hello I've yet to receive my sons pip 2 form but moving through it slowly on sheets of paper so not wasting this valuable time, but my question is regards question 2 A, do you just list all the medical problems here and date they started or also the effects of these conditions ? Thank you

Just the conditions and the dates when they started.
You will be able to explain the effects that your conditions have on you as you fill in the rest of the form.
