DLA to PIP Guides I am comfused

Hi All,
I am so confused with pip ! I have had the letter to ring DWP to change from DLA to PIP ànd I am so scared. I have tried to find the step by step guide to claiming pip in your members guides (I am a paid up member) but can only find sample guides with red writing over them.
When I have used your guides before for ESA & DLA Everything was so much easier to find ànd clearly labeled, but for pip all info seems to be about appeals , or challenging reports etc. I can't even find a copy of the form they will send me after the Initial phone call, so I can start working on it.
Please can you point me in the right direction on this as anxiety has kicked in ànd my head is all over the place.

p11 discusses the initial phone call and the information you will need to have at hand.
