In process of completing PIP2 form and wondered what phrase I should put if needs someone to complete the entire task for me? I have read definition of assistance, supervision and prompting but don't see anything that resembles the extent of help I need. Any advice very welcome.
Thanks

If you cannot perform the activity yourself then you need to explain why this is the case, you need to explain why suitable aids or appliances would not allow you complete the task and why prompting, assistance or supervision from another person would not help either.
Using an extreme example; if you did not have the use of your arms then it is unlikely that you would be able to prepare and cook a meal. The underlying reason why you cannot complete the activity would be the problems with your arms. You would be unable to use aids for similar reasons and no amount of prompting, assistance or supervision would help.
Make sure you consider the use of "reliably" and "on the majority of days" in your answers.
