ESA refused didnt fill in form very well

Hi I am trying to assist a relative who didn't fill out his ESA50 form very well and has just had his ESA stopped , he is going for the mandatory reassessment , I cant see where you get the opportunity to explain your condition in more detail or when you can send off another ESA50. I've read through the guides but I have very poor short term memory. Can anyone point me int he right direction? thanks

As part of the request for an MR you can explain how your relative meets the ESA criteria, filling out a new ESA50 may cause confusion but you can use the format to help you create the MR letter.

Make sure that the MR request is with the DWP no later than one month after the date of the Decision refusing their ESA.
