My full name is showing, how can I stop it?
If you don't have any recent evidence then there is not much that you can do about it.
Have you spoken to your GP about the issue, they may be willing to provide support or have ideas as to how you can negate the evidence problem.
Have your conditions changed since the last time that you had written evidence? Are your conditions ones where it would be reasonable to assume that they would remain stable or even deteriorate? If so then you can use this old evidence providing you can explain why it is still relevant to your problems as they are now.
