Hi ,my husband has received the ESA 50 questionnaire and the stress begins ,he suffers from degeneration of L4/L5 lower back injections and nerve block failed back at the hospital next week ,he also suffers with PTSD and with that there follows ,depression ,anxiety ,severe anger ,anti social basically anti everything ,so how do I fill this form in for his best chance of success and can I ask for a home visit there is no way I could get him to a face to face if that is what they decide do I drug him with diazepam I'm really stuck so any advice no matter how small or big will be gratefully received , Thanks.
