Hello hope all you wonderful mods are doing well 
Managed to complete the cooking and preparing question -the advice I was previously given by Gordon is -when I'm thinking about each activity to consider whether the problems I have would inpact on my ability to do another activity - so for example would it be sufficient at the end of the cooking and preparing answer to include a few lines saying " because of my physical disabilities and restrictions this also impacts in my ability to eat and drink, wash and bath and dress and undress reliably without physical assistance "?

Ye you can do this but the important thing is to explain your problems when you are answering the questions about these activities.

Ye you can do this but the important thing is to explain your problems when you are answering the questions about these activities.
