I'm a firm believer in presentation. With that in mind what's the best way to present additional information:- One Question per sheet? Two or three per page?
I'm inclined to one per page even if there is only one line on the page.
I simply printed all of my information out as one document, running each question on from the next. It is, of course, entirely up to you. As long as you make sure that whatever you submit can be easily accessed and scanned onto the DWP computer.
