Pip form- wrong boxes ticked

Hi this morning my CPN came to fill in a pip form with me. She's ticked a couple of boxes as sometimes which I disagree with. She said if I disagree and want to change it I can just put a cross through and tick the other box.. Does anyone know if this is right?
Or what happens if you only tick sometimes.
My background is bipolar disorder mixed affective state and ADHD. Recently diagnosed (last six months) but have battled with what was thought of depression and anxiety for a long time.
She's told me to fill in the rest of the form and explanations myself.
Has this issue meant that I have messed up my form?

I don't believe it will be a problem, but if you are still concerned, include a note saying that the original selection was done in error.
Have a look at our PIP Claim guide for assistance with completing the rest of the form and to ensure you are happy with what has already been written.
