I Had my home assesment 8 days ago and plucked up the courage to ring the DWP for the assesment provider report (PA4) but they still have not received it any advice is appreciated to what may be holding it up?

I'm not sure why but you have replied to a post from over two years ago!
The report could be subject to audit, a random percentage is selected each month, there could be problems logging it into the DWP system.
Have you contacted the Assessment Provider?
