Hi is there anywhere on the site that has a gp proforma that I could give to my doctor to provide medical evidence that I can send off with my MR? Also am I correct in thinking that I can submit this additional evidence after I send the MR (as doctor says cannot deal within the one month time scale) as long as I inform DWP in my letter?

If I understand what you are looking for their is a GP Notes document in the PIP section.
You can tell the DWP you will be submitting further evidence as part of your MR request, they normally allow claimants four weeks to do this, which from your post may not be enough time, all you can do is ask for a longer period and see what happens.
