Have got all my bits togetter for the form what I can anyway - my Doc does not do letters of support anymore but have a copy of the 2012 one so will send that again as it was a good one, my day centre Which I attend 2 days a week have given me a good letter to give to them so will put that in plus my one from them from 2012 as well t shows how I've bad I've got over the years, do you think this is a good idea? Also had new X-Ray's done last month which show my arthritis is now in my knees so it's all over my body now and that I'm seeing a specialist for my jaw next month and starting physio at my local hospital soon because how bad my legs are now - and a list of all the stuff social services have given and don't ne to my home to get about and make things easyer for me - Had someone who knows how to fill out the form correctly come out and do it for me and just hope that's enough to keep me in the support Group again...
