May I ask if there is a template to request the PA4 form and also just to check that it is indeed the PA4 that is made after the F2F assessment for PIP?
Apologies if I have missed the template but for the life of me I cant find it.
There isn't a template for this, you can either phone the DWP office dealing with your claim or write to them and ask for the PA4 assessment report. Phoning will be a lot quicker, if you write remember to include your NIN as a reference.
