PIP Renewal form - change in condition?

Hi, I've received my 'How your Disability Affects You' Renewal Form.
I was initially awarded enhanced daily living and enhanced mobility in March 2016. (I was never on DLA).

I would like some advice on completing the form without setting myself up to be penalised.
Nothing has changed as far as my daily living is concerned.
However, I have noticed a decline in my mobility. I have Fibromyalgia and osteoarthritis in my lower spine and currently being referred to my local CFS/ME clinic.
My walking is much slower, (I use a stick) and more painful and my energy levels are much worse. The smallest exertion takes a lot of effort and longer to recover from.
This means things have got harder on the walking and standing component (which I was initially awarded 12 points for. I'm, afraid if i tick 'harder' I am setting myself up for a f2f which I am dreading.

The other issue is, I scored 4 points on the planning and following journeys to unfamiliar places because of my depression and anxiety. My son or daughter accompany me. However, since May of this year I have a car on the motability scheme. I use my car for gp, local hospital and reflexology appointments at a local charity. These journerys are less than 15 mins, if I have an appointment which is further away or I've never been to, I need someone to come with me.
I'm not sure what to tick for the planning and following journeys. It hasn't really changed for me except I don't need to wait hours for dial-a-ride, taxis or hospital transport.
I appreciate your advice .

The PIP guide has advice on filling in these forms. We have a few members who have undergone reassessment for PIP but we haven't really seen any consistency regarding who has to have face to face assessments.

If it is any help, I replied 'worse' for some questions and my benefit was increased without a f2f.

Can I ask if you sent in more evidence to say things had worsened or did they just accept it without a F2F Mrs H? Many thanks

I don't think I sent in anything other than a prescription form that showed medication for a new condition.
