I attended my Health Assessment in April. Having moved to Bath this was the third time as Bath is an area of the country trailing ( and what a trial! ) Universal Credit, so of course they were able to drag me thru all this yet again. At the end of the assessment when asked if there was anything I would like to add, I asked if she had a copy of the response from my G.P ( its called a UC113 but I guess ESA will be called ESA113 ) as I had not received it in the paperwork from the DWP. She got quite flustered looking thru her papers and I explained that I had a copy and did she need to photocopy it. The mask then slipped and she got a right cob on demanding to know how I had got a copy of this UC113 and I explained that I had phoned my G.P's surgery and asked the administrator if they had received this form and whether my G.P had filled it in. She said he had and I popped in an got a copy of it. Of course the 113 totally supported me and my claim. The Health assessor had ignored it ( she found a copy ) and sat grim faced as I went thru the 113 using it to back up what I had said and my claim. Of course it was not mentioned in her report, like it did not exist. I am only telling you this to advise others to phone your G.P and ask if your doc has filled in the form and ask for a copy.
