There's no reason why she should not sign it as a doctor, but there are two issues that she needs to be aware of.
First of all the panel can only consider your conditions and limitations as they were at the time of the Decision being appealed so anything that she writes must make it clear that this is the case.
If she signs it as a doctor then she must explain that her report is the result of a formal review of your conditions, as she would do for any other patient.
