I have completed the telephone interview and received my form to complete. However, I note from your guide that I should have received a copy of the form completed in the telephone interview to sign and return? This hasn't happened.

The form is now only sent out if there are outstanding issues from the call, so if you have been told that a PIP2 will be sent then you should not expect to receive a copy of your PIP1.
