hi i am unable to find any info on filling out the new PIP review form. I can only find info on how to make a fresh claim have i missed something ?

My full name is showing, how can I stop it?

There is a section in the in the PIP Claim guide that covers the AR1 Review form, you are being assessed against the same criteria as when you completed your PIP2 form.
