I would like to download the PA4 form

I’m having an PIP assessment at home the first week in April, a McMillan person is also attending with me. Very concerned as they can have a diabolical attitude. I suffer from arthritis, fibromyalgia and to top it all jaw cancer operstion 2 yesrs so eating is a problem. I can drive and need my car as I can’t walk far for long. Can they still take your car away if it goes to Mandatory appeal. Plus I have a carer twice a week how do I show them prove, obviously don’t want to give them the bill as proof. I’m a pensioner also.
