Your restricted diet is not going to help you score points, you need to concentrate on the reasons that you need prompting to prepare and cook a meal and to take nutrition.
So what conditions is that mean you need the prompting? How do they manifest themselves? Are you effected differently throughout the day? Are you effected each day in the same way?
You don't need to answer these questions here but you will on the form if you don't tick "no change".

Most days im to stress to think about eating if someone cooks me something and something in there I dont like I get very anxious and cant eat it. Most days I.only eat one meal after a text reminder saying have you eat or make yourself something small
Don't forget to explain what would happen if you did not receive that text, remember those assessing you will be expecting hunger to motivate you to eat!
