What forms to request after F2F ?

What forms to request after F2F ? was created by fastmoggy
Sorry guys, been a few years since last battle with DWP but we had a F2F at home and now await the DM decision. If things go wrong, what forms and names for them do I need from them? by that I mean the F2F report and the decision makers decision. As ive never had to contemplate a mandatory decision application I just want to be 'up to speed' with the current regulations.

You don't mention the benefit but I assume this is in regard to your PIP claim?
The only thing to request is a copy of the assessment report (PA4) but you can do that now you do not have to wait for a Decision, just contact the DWP office dealing with your claim.

You don't mention the benefit but I assume this is in regard to your PIP claim?
The only thing to request is a copy of the assessment report (PA4) but you can do that now you do not have to wait for a Decision, just contact the DWP office dealing with your claim.

thanks, yep and yes it was for PIP.
The PA4 report, all coming back to us now.
To be honest we have a good feeling about this one? as the interview lasted less than an hour as opposed to the just over 2 hours one we had to endure with devastating results point wise.
Mind you on the other hand???? time is going to tell when the brown envelope arrives .
