Since I became ill 5 years ago and started on ESA, I have been lucky enough to have the same GP and Consultants. Unfortunately, my GP has left the Practice. I have not seen any other doctor in this time as the GP I saw knew me inside out. Do I put a note on the form to this effect? I am not going to get an appointment just to update another doctor on 5 years of consulations for ESA purposes. I just see my 2 Consultants on a 6 monthly basis and talk to them instead. Thanks
