UC50 Form

Hi

I signed up a few weeks ago and have sent off my PIP 2 form using the guides on here (which were very helpful) and applied for Universal Credit which was awarded as standard rates. I haven't received the UC50 form yet. I handed my sick note in on the 22nd of October I think and my understanding is that it should have been sent out 30 days after handing my sick note in, although I'm not sure how long it takes them. Should I phone the UC helpline and ask them to send it or the job centre I'm based at?

You should have been forwarded to Maximus four weeks after you presented your first Fit Note, I would chase your JC+ advisor via your Journal for an update.
