They should be using the information from your original claim, as a minimum, this will include the assessment report, your PIP2 and anything you attached to it, however, I would still request a copy of the report as it will allow you to see where your arguments were successful and not so that you can adjust the replies you make on this form.
Whilst a home assessment may be offered again, you really need to preempt this by getting your GP to write to the Assessment Provider explaining in unambiguous terms why you cannot attend at their offices.
