Unfortunately the answer is no you will need to complete the form if not then your claim would stop. Did you make a copy of the previou form? if you did then use same information on your new form if your condition has not changed.
A quick thought is the form for the same issue i.e PIP or ESA. We are coming across many cases where you get assessed for PIP only to be sent new forms for ESA and vice versa.
