Hi Gordon/other moderators
Just a few questions,
1. how long does the PIP2 form take to arrive after the PIP1 phone call?
2. I want to request my recent paperbased ESA assessment document. Is this an ESA 85A? How long does it take to arrive.?
3. Is there any definition of the word familiar re familiar journey? I live in the middle of my town and can manage a simple route to the shops. However there are lots of journeys (that I have done with my partner 100s of times over 10 years) that I cannot manage myself because I wouldn't remember the route.I have Mild Cognitive Impairment which causes many issues including spatial awareness and visual memory.

Times are only approximate.
1. 7 days or less
2. 7 - 10 days
3. Familiar journey is one that is known to you - which you may be able to follow by yourself or it may be one that is known to you, but you would not be able to follow without help. So from what you said below although you may have followed a certain route on many occasions this is not something you can do yourself without the support of your partner. (You need to explain why)

I'm still a bit confused. so if I say that I am able to do 3 very simple routes near my home on my own yet and can't do most routes I have done many times without the help of my partner that counts as not being able to do familiar routes on the form? How is it best to word it?
I'm also confused re the planning a route descriptor.What is meant by planning a route? Using a paper map or googlemaps?

I cannot tell you how to write, but the difficulties that you describe are very clear. Use what you have said here. Here is a breakdown of the Going Out activity for PIP
Planning a route - this is primarily a cognitive or sensory (e.g. blindness) activity. You are being asked about the problems you would have with working out how to get from one place to another, you do not need to be able to follow the route that you are planning.
Undertaking a Journey - this is to do with mental health issues such as agoraphobia and social anxiety and is concerned with you leaving the house to go somewhere, they will be interested in the things that stop you doing this. You need to show that you would suffer "overwhelming psychological distress" to meet the criteria.
Following a route - This activity about the problems you would have navigating a route. So are there problems; cognitive, sensory or mental health issues that would prevent you from doing this? This is different from undertaking a journey, in fact, if you cannot undertake a journey then you will not score points for following one and vice versa.

12 weeks after receiving the text saying that my PIP2 form had been received I have yesterday been sent another text from dwp saying the following " we have received the written report of your PIP assessment. We will write to you once we've made a decision on your PIP. As a guide you should hear from us within 6 weeks."
Does this mean I have avoided a face to face assessment.? Would it be wise to request a copy of this report now? Does such a report have a title to ask for (like the 85A ESA one)?

Yes, you have avoided a face to face unless the DWP decide the report is not sufficient but this would be extremely rare.
You should be able to just ask for the assessment report by contacting the DWP office dealing with your claim, the official name for the report when no face to face is done is a PA3.
