Hi I am sure this has been covered elsewhere but how do I complain about the nurse who blatantly fabricated my wife's assessment. When she asked for a mandatory reconsideration which was not found in her favour she was told in writing that she should take up her issues with the assessor. Which would be a complete waste as she obviously would not say that she misrepresented the assessment even if I new how to contact her. Any advice/assistance would be appreciated. Many thanks
