Please could you advise me whether there is a template on this site for writing to a consultant to ask them to provide me with medical evidence for my PIP claim and where I might find it. I have looked in the guide and cannot find a copy. Thanks
In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ
There is a GP Notes document in the PIP section your Consultant can either complete it or use it as a template for there own letter. You only have to give them the pages that you think are relevant.
