2nd esa50 received despite original being received

hi, i received the dreaded ESA50 from the Health Assessment Advisory Service in MAnchester , it took about 8 hours to gather all my evidence & complete the form. I sent it in an rang ESA who confirmed it was received on 23rd September , I also rang the Assessment advisory service who confirmed that it had been received on the same date but ESA had requested that another be sent out a week after the orifginal being received. I returned this uncompleted confirming i had already returned this. The ESA advisor said there was nothing on record as to any reason why the 2nd had been sent. Have you ever heard of this before & can you advise me why. This is causing even extra stress & anxiety

I can only guess that it was an administrative error by HAAS. It is certainly not normal to have a second form sent and I can't remember a case where it was.
