PIP form filled in by friend

My friend filled in the PIP form as I have a problem with my shoulder and it becomes very difficult for me to write. I stated on the form that my friend has filled it in. I have signed form. Do they have to put their name down and sign it as well?

Thank you Gordon. I have stated they have filled it in for me because of my shoulder pain, I have difficulty writing. I hope this is sufficient.

I dictated the answers to them and they wrote them down.

My husband filled mine in for various reasons with me telling him what to write and I signed. However he also wrote a covering letter to explain he had filled in the form for me with my instructions. We both signed the letter, I didn't have any problems.
